All volunteers must complete both the Volunteer Confidentiality Statement and the Background Check Authorization forms (attached under Downloaded Files on the left side of this site page) and submit the originals to the Dept. of Family Protective Services (DFPS) five days prior to the event (Sat. Sept. 19th).  If you need to fax information to the DFPS prior to sending the completed originals, its fax # is 817.276-3967.
