To obtain a Rotary license plate from the State of Illinois, simply download the application by clicking on the Rotary License Plate Request Form under the heading of "Download Files" on the home page.
The Secretary of State's office requires a minimum of 850 requests before we can design the actual plate. There is a $25 upcharge for each plate each year. In the first year, $10 will go to The Rotary Foundation for each plate. Thereafter, $23 will go to The Rotary Foundation for each plate. A person does NOT have to be a Rotarian to purchase a plate.
